Add a comments/report section to the Rate Submissions section

Question

After rating some submissions, it is clear that some kind of comments section, which could be very similar to the comments section on the course questions, could help users to improve submissions which are close to being good submissions, but may be containing some errors. An additional report section with a back end points system to signal any users repeatedly submitting incorrect questions and answers would benefit the moderators. Using the comments, the OP could then edit their submission.
