Getting - Template Not Found - Error in screen

Question

Hi Guys, I had developed a screen with HTML, CSS and JS. In my screen, I am getting trying to display like button control with caption in it. In the place where button has to come, I am getting "Template Not Found" as shown. If anybody know where I might have done mistake, pls reply to this post.
