how to reference php code in html

How can i referece my php code to a html code i have created in solorlearn. getting the script name and putting in the action tag in the form tag. sorry if my question isn't clear THANKS IN ADVANCE

No, he’s asking if you can reference another of his PHP codes on SoloLearn and put it as the action attribute for the <form> tag.

yes i knew of puting it in html but just wanted to know if i could seperate it as with css and javascript. THANKS FOR ALL YOUR ANSWERS
