Why is all my php code showing when I click submit button

Good day here, I wrote HTML and PHP code . the PHP code is to collect data, like if I input username and password but when I click submit button, its the PHP code dat always show up. Even I had specify the action and method. Pls help

Save a copy of the code as PHP code bit in SoloLearn, and attach its link in your post's Description. Without a closer look it'd be difficult to tell what or where.

Additional information that may help contributors, you may also add these in post's Description:

- What software was installed to host PHP e.g. xampp, wamp etc.

- File extension used for naming the PHP code.
