If you are talking about documenting your code, don't document the obvious, but make notes about things that aren't understood, like motivation for doing A over B, or explaining how things should work vs how they do work. This kind of documentation is "Beyond the code" and helps understand the human element for why the programmer did what they did, also it helps me to remember why I did what I did. Oh, and always document bug changes, it saves so much time!
