Advanced insert and filtred search in php

hello, please someone if can helpe i need a script for (mistery client) ho most go in the storey and see all the price and then insert in to a table and than the admin most see the hole table created in mysql and filter the result by article name, store, year, mount, day, and name of mistery client can someone helpe please?
