Do you need to know maths perfectly to be a programmer?

Not perfectly. I think that is impossible. But if you have no concept of math (also seemingly impossible) then it would be hard. I think it is beneficial but not necessary.

Not at all but as you become more advanced it will be an asset

Thinking like a mathematician will help you alot.Basic maths like modulus,division and basic maths laws will also be needed.
