Hmmm, Its possible to forget password but I doubt if a person can forget his username more especially email address.
I can help you but I cant because am not sure if its your own account you want hack.
In programming I was told, if you want log into your account, you will say, 'I want to access' but if you want log into someone's account, you got to say, 'I want to hack'. Anyway if your sure its yours I can give you a couple tips.

why not just create new email and social profiles... I mean if you haven't used it for so long it's kind of like starting fresh anyway.
