PHP is not made for Web design, it's made to manipulate data before displaying them. If you want to do webdesign, you can learn basics of design (to forge your culture of design and train your "eye"), photoshop/illustrator or any other drawing software to" draw" web designs, and front-end development languages to actually put webdesign in place : HTML and CSS. And if you want to make your design interactive (= react when the user perform an action), Javascript is needed too. Try the HTML, CSS and Js courses on sololearn, they are a very good start. (and then, PHP would be used to inject content in your pages)

HTML is client side language which interact with only with your browser and PHP is server side programming language which interact with server.html is a markup language with HTML you can create static web pages and with PHP you can create dynamic web pages e-commerce sites etc.
