How can one differentiate between an app and a websites?......Since most websites have same layout with some apps and of course built with same procedures and tools.

Hmmm, today that's a really blurred line since we're capable of web applications now. Most applications are not websites, but many websites are or contain apps embedded in them.

App = application
Website = document you view via browser
Web app = application that's embedded in a website

That's basically the idea behind it. Often when you hear app by itself, it's referring to an application on your device that isn't access via a website, but rather from the device itself.
