Blockchain and php

Question

can someone please explain how this works.. I want to save and retrieve patients data with PHP (so it's gonna be a web app). but I still don't understand how blockchain can come in.

is block chain some kind of framework or concept?

The basic concept of blockchain is quite simple: a distributed database that maintains a continuously growing list of ordered records.
