You just create a bunch of stuff to show off your abilities. For many people, they'll create their own personal website to showcase it and make it easily accessible. For some others, they may have it on a drive somewhere.

Either way, a portfolio is basically you showcasing what you're capable of so that you can present/prove it to others, usually potential employers.
