Save the code and attach code URL in Description rather, looking at the code this way is way too hard to read, let alone to debug.
BTW undefined index error mostly be triggered by the use of invalid array index or key, so the best I can suggest you right now, is to do the following;
print_r($retrieve_image_check);
Then verify, whether an element identified by "DOJ" key exists, I suppose you can start from there.
